Forman Problem Gambling Indicator (FPGI)

This questionnaire is a self-help test and therefore may not be reliable if taken for someone else. Items were developed by Forman Consulting in conjunction with material culled from the American Psychiatric Association’s Diagnostic and Statistical Manual IV, the South Oaks Gambling Screen and Gamblers Anonymous’ 20 questions. The FPGI weights items subjectively and therefore is not a scientific instrument.

Many people who have sought help for a gambling problem have reported experiencing at least one of the above symptoms. Again, the FPGI is not a scientific diagnostic screening tool and ought not be used as such. However, in many cases, responding in the affirmative to several or all questions may indicate a gambling disorder.
